Patient Navigation Services
SHARE is excited to launch our Patient Navigation Program for patients diagnosed with breast and gynecologic cancers. Our goal is to help end health disparities by addressing barriers to quality care. We invite physicians to refer their patients to SHARE’s certified Patient and Financial Navigators, who can provide a full range of tools and services.
What is Patient Navigation?
Patient Navigators offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care through all phases of the cancer experience.
SHARE’s Certified Patient Navigator works directly with referring physicians and patients to identify medical and psychosocial barriers to care, provide psychosocial distress screening, advocate for patients’ needs and assist them in accessing the appropriate local and national resources, provide appropriate interventions and resources and coordinates with the clinical health care team to discuss patient progress, barriers and services needed.
What is Financial Navigation?
Financial navigation helps patients understand their out-of-pocket expenses and what their health insurance plans may cover. Financial navigation may also help patients set up payment plans, find cost-saving treatment methods, and improve access to health care services that the patient needs.
SHARE’S Certified Financial Navigator assesses the patient’s financial concerns and coordinates with appropriate resources, assists patients with financial paperwork required to access Medicaid, Medicare, Social Security Disability, and other community or national assistance programs, provides patients, families, and caregivers local, national and government financial resources, helps facilitate understanding of insurance coverage specific to their cancer treatment.
